adding Rate this app message for your App ?

I’m trying to add a message option to my app, I want to the user will able to Rate my app , when the click will open the App store ( Apple store, Play store, Amazon Store, Nook store) .

Can somebody help me how can I do that ? what’s the codes I need ?
