Any tutorial on PHP/registration type app

Hi all, wondering if there is any good tutorial out there for uploading a few text fields to a database from the app that can then be viewed in a web browser? For example User enters first name last name and email and clicks register (within the app) that data is then saved in my (lets say godaddy SQL database) for viewing on a desktop browser.
