Maybe there is something wrong with the icon file then. Like your icon editor is doing something non-standard with it.
Try grabbing a pre-made *.ico file somewhere else and give it a go… for testing purposes.

Just click “Report a Bug” at the top of this page, reference this forum thread and my name “Joshua Quick”, and attach your *.ico file. I can give it a quick go on my end.

You should have gotten an email confirmation from the bug submission. In the subject of that email is a 5 digit case ID number. That’s what we need.

Also, I’m pretty sure there must be something wrong with your *.ico file. Perhaps the icon editor you are using is doing something non-standard with it. Try a different icon editor. What I’m getting at is don’t wait for us. Everyone else has managed to add their icons to their Win32 apps so far. ;)
