I’m not sure, but I don’t think Corona Labs would extend your subscription by 6 months just because you filled in a survey? Maybe they picked one winner of all the surveys?

As I understand it, that form was to signup to be on the Corona Labs survey team, where you would be asked regularly to complete surveys and help guide the direction of the development of Corona.
