Inneractive ad problem ICS / Galaxy Nexus

I have banner ads setup and being tested on an app. It seems to work ok on Android on some devices tested (Xoom, HTC EVO 4G tested), but on Samsung Galaxy Nexus with ICS 4.02 it is currently display only a box with a ? (question mark symbol) inside.

Note: this is happening most of the time, but not always. It may even be related to adult content of the ad?

This has been observed on more than one Galaxy Nexus.

The ad is actually still active. This means that you can still click on it and be taken to the market to download the app that was being advertised.

Anyone else seen this?
Any ideas how to fix this?
Or, is this just a problem for Inneractive to fix?

Yep, the question mark shows up sometimes (I have a Samsung Galaxy S II where it showed up often last week, it’s better now. A friend in Europe is testing for me and he has a iPad, there he currently sees the question mark frequently)

I don’t think it’s missing fill rate, because the ad underneath is active - just the image was maybe not transferred correctly or something along those lines.
